Having a good reputation is priceless. Being the "difficult patient" at your doctor's office is worse than having a bad credit rating at the loan office. Flawed medical records are particularly detrimental for patients with chronic illnesses or persons in the process of obtaining new health insurance coverage.

In California, adult patients have a guaranteed right to create an "addendum" (limited to 250 words) that must be attached to their medical chart. A health care provider has no right to refuse a patient's "addendum" even if they disagree with the patient's version of the facts. (Health & Safety Code ยง 123111(a))

Federal law (the HIPAA Privacy Act) also allows patients to request "amendment" to their "personal health information" (PHI). Federal rules do not restrict the number of words to be added or changed. However, the patient's request for making corrections may be denied if the provider believes the PHI "is accurate and complete." (45 CF 164.526(a)(2)(iv))
